Eligibility

To be eligible for services at the Chippewa Valley Free Clinic, several criteria must be met:

 

  • You must be uninsured or under-insured. You cannot have Medicaid or Medicare/Badgercare or be eligible for any other government programs, such as VA benefits or Disability.
  • Your household income must be at or below 250% of the federal poverty level (FPL). See the table below.
  • To confirm your eligibility, you must provide proof of income for yourself and your spouse (if applicable). This proof will be a tax return and paycheck stubs (2) OR Social Security Statement, and/or unemployment statement.
  • You must reside within our coverage area, which includes the following counties: Eau Claire, Chippewa, Dunn, Pepin, Buffalo, Trempealeau, Jackson, and Clark County. Proof of residency is required.  We encourage those living in Chippewa, Dunn, or Clark counties to visit the free clinics in their areas. Click HERE for a map of clinic locations.  
  • Exceptions are evaluated on a case-by-case basis.
